How do I request an appointment online?

To request an appointment online, simply click Appointment Requests (above) and login if you have already created a profile or create a profile if you are a new visitor. Then choose a location from the drop down menu, and click Continue. Complete and confirm the information listed and click continue to submit your request. A staff member will consult our physician's schedule and contact you with an appointment.

Can I request a prescription renewal online?

After creating a profile, patients are now able to request a prescription refill online by clicking on our Prescription Renewal tab, located on the top of this page.  Complete and confirm the information listed and click continue to submit your request. A staff member will contact after processing your request.

Can I request my medical records and x-rays online?

After creating a profile, you can quickly and easily request your medical records online.  First click Request My Records (above) then click on "All patients requesting copies of their MEDICAL RECORDS or X-RAY FILMS click here".

Can I pay my bill online?

Pay my Bill Online allows patients to conveniently and securely log into their patient page and make an electronic payment on their balance. Patients also have the option to retain their information on file and set up an online monthly payment plan.

Should I complete a patient satisfaction survey?
