Where is Blair Orthopedic Associates located?

We offer two convenient locations at 3000 Fairway Drive in Altoona and 121 June Drive in Roaring Spring.

How do I make an appointment?

We now offer two convenient options for patients to request an appointment. We invite you to try our new online "Request an Appointment" above or call us at  (814) 942-1166.

What items do I need to bring for my appointment?

We ask that every patient bring their insurance card(s). If your insurance requires a referral or a voucher you must bring that information as well. If you are unsure if this applies to you, simply call the telephone number located on the back of your insurance card. You also must bring means to pay your co-pay and deductible.

If you have had any testing or films taken for your current injury/condition, please bring them along with the physician’s report to your appointment. These results will be helpful to our physicians when caring for you.

Are there wheel chairs available at your locations?

Wheelchairs are available at all of our locations.

What are my financial responsibilities?

Your insurance company requires us to collect your co-pay at the time of your visit. Your co-pay amount is subject to change depending on your insurance plan and any procedures that may be provided during your visit. If you are unable to pay your co-pay at check-out, you will be responsible for a $6.00 billing charge. If you are unable to keep your appointment, you must cancel 24 hours in advance or you may be responsible for a $20.00 no show fee. All outstanding balances must be paid prior to your next visit or you might not be scheduled until payment is made. If you are a workmans’ compensation patient who refuses to provide your personal insurance as a back up and your workmans’ compensation insurance is denied, you will be personally responsible for payment.

Do I need to bring a referral to my appointment?

You are responsible for managing your insurance. If your insurance company requires a referral, you are responsible to have your Primary Care Physician send a referral to our office in advance of your appointment. If you have any questions regarding your insurance coverage or eligibility, you should call the toll free phone number located on the back of your insurance card.

How do I request a copy of my medical records?

To obtain a copy of your medical records, you may simply submit a request online by clicking "Request My Records" above or you can call our Medical Record Department at (814) 204-1186. We ask that you allow seven to ten days after we have received your information to complete your request. We will contact you via telephone and/or email as soon as your medical records are ready to be picked up at our office. We will let you know if you need to sign an authorization to release your records.
