If we participate with your primary insurance plan, we will submit insurance claims for all services.  However in most cases you are responsible for:

• Co-payments at the time of service
• Deductibles
• Co-insurance

If we do not participate with your primary insurance plan, full payment is requested at the time of service.  As a courtesy we will submit a claim to your insurance company for all charges incurred.
