WORKMANS' COMPENSATION CLAIMS
 
 

First time workmans' compensation patients must bring the following information:

  • Date of injury
  • Most recent work status
  • All related medical records including x-ray films and office notes
  • Medication/list of prescriptions
  • Employer information (address, phone number)
  • Insurance information (address, phone number)

Contact your human resources/personnel department to obtain insurance and employer information.

Although your injury/claim may be work related, we will ask for your personal insurance information.  If you refuse to provide us with your personal insurance as a back up and your workmans' compensation claim is denied, you will be personally responsible for payment.

Helen Carper, Workers’ Compensation Liaison, has been a part of the Blair Orthopedics team since 1997, and in the worker’s compensation department since 2001. In most workers’ compensation cases, she will be your first contact at Blair Orthopedic Associates. Helen is responsible for scheduling worker’s compensation appointments that are referred by occupational health facilities, family physicians, employers, insurance companies, nurse case managers, other specialists, and patients, that are of a non-urgent matter. All urgent care appointments are scheduled through our operators and nursing staff. Independent medical exams are scheduled through our Patient Relations Supervisor at extension 231. Helen is also available to address any concerns or questions you may have, and to communicate with the doctors on any issues you may need answered by our physician staff.  Helen’s office hours are Monday through Friday from 9:00am until 2:00pm.  Here at Blair Orthopedics we strive to make the worker’s compensation process as easy as possible.

 

Crissy Weyandt, Worker’s Compensation Liaison, has been a part of the Blair Orthopedic team since 2002. Crissy is available to help answer any questions that the injured worker or our physician(s) may have during most workers’ compensation appointments.  Her goal is to obtain the most accurate and up-to-date information, regarding the patient/injured worker’s plan of treatment, testing, therapy, possible surgical intervention, and the current work status along with their re-evaluation date.  Once her information is collected, she enters it into our computer system and sends the information to the injured worker’s employer.  In addition, if the patient was referred to us by an occupational health facility or if they have a nurse case manager and/or an adjuster, the information will also be sent to them upon request.  Her office hours are Monday thru Friday 8:15am -5:00 pm.   Thank you for choosing Blair Orthopedic Associates & Sports Medicine to take care of your workers’ compensation needs.
