• WHAT TO EXPECT

    Prior to your first visit, it is important for us to review your previous medical history.  Please arrange to have your medical records from your current health care provider forwarded to our office and bring any x-rays or MRI films that may have been taken. Also, please bring a list of medications prescribed and non-prescribed.

     

    To save you time, we will try to mail you our patient registration forms.  Please complete these forms, at your convenience, and bring them with you to your appointment.  If you have not received our patient registration forms via mail, they are also available online by clicking PATIENT FORMS.

     

    If you have not had the opportunity to complete our patient registration forms prior to your appointment, please arrive approximately 15 minutes prior to your scheduled appointment time to complete our patient registration forms.

     

    Upon checking in for your appointment, we will offer you a copy of our NOTICE OF PRIVACY PRACTICES.  It describes how we may use your health information and outlines your privacy rights under HIPAA, a federal regulation governing patient privacy.  We will ask you to sign an acknowledgment that you have been offered this notice. 

      

    Each time you check in for an appointment, we will verify your address, telephone number, and insurance information.  Please bring your most current insurance card to each appointment.  Be sure to bring any referral forms required by your insurance plan. 

     

    At the end of your visit we ask that you stop at one of our check-out desks to pay any co-pays and to schedule your next appointment if needed.


  • STAYING ON SCHEDULE

    Even though we make every attempt to stay on schedule, many factors can disrupt each day. You can help us stay on schedule by doing the following:

    If you have not had the opportunity to complete our patient registration forms prior to your appointment, please arrive approximately 15 minutes prior to your scheduled appointment time to complete our patient registration forms.

    When you need cancel or reschedule an appointment, please notify us as soon as possible so that we may schedule another patient who is waiting to be seen.  We prefer at least 24 hours notice. 

    All appointments must be scheduled.  We do not see walk-in patients.

    Please do not ask your healthcare provider to evaluate an additional problem or family member that are not scheduled for that particular appointment.

  • TREATING MINORS

    In most cases, patients under the age of 18 must be accompanied by a parent or guardian for every appointment.

     

    In such circumstance that a grandparent, family, friend, or baby-sitter must bring a child to an appointment, the below form must be completed by the child's parent or guardian and sent with the child to that appointment:
